NEW PATIENT PAPERWORK

In order to serve you better, and reduce the time you have to spend in the clinic, we ask that you handle your new patient paperwork BEFORE your first appointment with the doctor.   If you can have the paperwork finished before you come to your appointment, when you arrive, we can go straight to your consultation and examination.

Step 1:  Download the forms (by clicking on the small icon below). 

Step 2:  Print the forms on your home printer.

Step 3:  Fill out the forms (as completely as you can).

When you're finished, you should have five pages completely filled out.  Please make sure to sign and date the bottom of each form.    Thank you for your cooperation.  Please bring the new patient paperwork forms with you during your first visit.  We look forward to serving you soon!
